2. Affiant Information: Full legal name and current address of the person making the affidavit, including their relationship to the person whose birth is being verified
3. Subject's Birth Details: Complete information about the birth including full name, date of birth, time of birth, place of birth (hospital/institution name and city), and gender
4. Parents' Information: Full legal names of both parents, their citizenship at time of birth, and their addresses at time of birth
5. Registration Details: Information about the original birth registration, including CPR number if available
6. Verification Statement: Statement verifying that all provided information is true and correct to the best of the affiant's knowledge
7. Signature Block: Space for affiant's signature, date, and notary/authority authentication
