Affidavit Of Parentage

1. Assisted Reproduction Statement: Required when the child was conceived through assisted reproductive technology, including details of the fertility clinic and relevant consent forms
2. Genetic Testing Declaration: To be included when genetic testing has been performed to confirm parentage
3. Previous Parentage Proceedings: Required if there have been any prior legal proceedings regarding the child's parentage
4. Military Service Statement: Required for cases involving active military service members, especially for overseas deployments
5. Posthumous Declaration: Special section required when establishing parentage after the death of one parent
6. Interpreter Certification: Required when the declarant needs an interpreter to understand the document
